How do I enter a note in Lightning?

How do I enter a note in Lightning? The most basic function of a CRM and it's not on the screen out-of-the-box.

 

Following the instructions that another use left on Trailblazer did nothing. I can see my old notes from Classic under Notes & Attachments, but there is no way to enter a simple note. Please help!

    Hmm...I do not use FSC so I'm not quite familiar with it. Can you post screenshots of your setups and maybe I can further assist?
