There is an Activity with Opportunities report type but it's not going to show you that the Opportunity was created as a result of the Activity. It sounds like that is what you need to see. Is that right? If so, you should do some customization to the Opportunity page, add a field for example, that keeps track of when Optys were created as a result of specific activities. The standard report type won't expose that to you out of the box.
