Sandbox is your testing environment and does not contain the real time data. To test an application recommended practice is to install an app first in Sandbox check for any custom code side effects and app capability to meet business needs. If all is good then install the same app in production and start using it.
I would say there is a huge difference in a way that whatever actions you will perform in Sandbox will not be automatically carried over to production. They will have to reconfigured from scratch. Also Sandbox contains clone of Production data to a point of time and is not real time you will have to refresh it to get the latest production data in sandbox. Any changes made to data in sandbox is not automatically reflected in Production and vice - versa. Also once you refresh your sandbox all the installed apps and customizations are lost if they are not in production.
