Since you have asked this question several times already it seems you may need to use the solutions already presented. Please take a look at the Appexchange options for Survey http://appexchange.salesforce.com/results?type=Apps&keywords=Surveys as some of them a free to install and use. Again if you insist to go down the Visualforce route you will need to post on the Developer Discussion Board as this forum is not the technical developer forum while the Developer Discussion Boards are.
