This is a really basic question, but can anyone tell me how to add Created By, Created Date and Last Modified date to the page layout for fields and relationships in Object Manager in Lightning?

Kelly is right--you cannot modify page layouts in Setup. My suggestion was for a workaround. You can store/view this information on the field record itself so you at least have access to it, even though that means you have to click into a particular field to find those details. There is no way you can see created/modified information in the list view.
