On Account record, my related list of Opportunities does not reflect the assigned sort order from the page layout. Is this a known issue or am I doing something wrong?

That's correct, user sort preferences are sticky in Lightning experience, for the 'View All' page as well as related list cards within the parent record detail page.
In Classic only the 'View All' page would have sticky user sort.
