The flow output fails intermittently for no apparent reason (no errors, etc and runs fine at times also). Flow run monitoring is enabled with high visibility. The flow failed and there is no warning appearing in the dashboard. I've seen this work as expected in other workbooks, why might this not be working as expected here?

The dashboard data source can be independent of a flow, which is probably the mode that Tableau is in - I'm just trying to explain why you might be seeing the error in other vizzes but not this one.
Its easily triaged though - just open a new sheet, and in the title replace this with the last data load field in the r/click menu and you can validate the update date against your flow. My guess is that this date will not be updated with the flow run date, which will tell you that its extract connection and needs to be updated manually.
