What needs to happen for an opportunity appearing on a campaign record? Does the contact associated with both the campaign and the opportunity have to be the Primary Contact on the opportunity? And when the opportunity _does_ appear on the campaign, does that take into account whether the contact is marked with a campaign member status that is a "response" or not?

Campaigns are added to opportunities when a campaign is related to a contact that is assigned a contact role on an opportunity prior to the close date of the opportunity. Campaigns and opportunities dont have a relationship directly, so with opportunity contact roles and this functionality you can 'link' both.  So if a contact is a campaign member and that member has an opportunity record created - It will automatically populate the Campaign Name in the "Primary Campaign Source" field.
