Evaluate the rule when a record is created, and every time it’s edited
Immediate workflow actions: create a task
Thats it.
Running on opps. It has fired on some opps, but not on a few, and will not fire on these opps regardless of what I do. Opp owner is active, account owner is active.
Any ideas on what to look at? Tried debugging in the dev console per this blog post from Vertical Coder, but there was no entry for this WFR that I could find.

Tried to export the debug log to look at it further, but cant figure out how to open it properly in excel

Just to confirm, Does the Opportunity is private(I.e Private chekbox on the Opportunity is checked)?
