Not all the stages are appearing in the opportunities pipeline report

Question

I have recently added more stages and replaced some, in the opportunities pipeline.

I have run a report and not all of the stages are appearing, even those with opportunities attached.

Also, not all of the contacts are appearing under the opportunitites.

How can I get all of the opportunites and their contact to appear?

Can you elaborate about "opportunities pipeline report" ?   I do not see it in new report type.
