Export Survey Responses #Salesforce Surveys

Question: Can export survey responses from Analyze tab and looking to add few more fields from Associated object (Lead). Is there anyway we can add extra fields (right now - there is visibility of  "Name" in CSV file) in Associated object?- Using Salesforce Standard survey package.

Question 2: Can create custom report type but difficult to add survey questions on headers row. Suggest me the possible solutions here. Please find the expectation image attached. Thanks.
