File Upload with Description Screen Flow

Hi All,

I am trying to create a screen flow triggered from an action on a custom object that will open up a screen allowing users to attach 1 or more file/video with an title and description called 'Issue title' and 'Issue description'. when saved it will attache the files to the record and if the users checks the file deatils the information from Issue Tiele and Issue Description are there. I have already created the 2 text fields on the content version object.
