Flow to create a new contact role when checkbox is marked true.

I am trying to create a flow when a check box on the Opp is marked true. 

I have a flow and when I debug, it works but when I try it after activating... the new contact role is never created. I know this may be hard to troubleshoot on here but I truly appreciate any help.
