The "Allow Access" button doesn't do anything when clicked - I just see more of the same screen. In contrast, if I click "Deny Access" panel changes to an error "custom_prefix.my.salesforce.com refused to connect."
I've tried restarting which didn't resolve the issue. And I haven't made any browser or Salesforce changes that I'd expect to affect the integration.
Any ideas?

Ok found it - it was an email directly to me that never got posted in the case on the portal. Here is what they said, and it worked...
This specific behaviour can happen when one of the flags for Gmail Integration in the Authenticator Provider object was not set correctly. When Gmail Integration queries for the token the result comes back empty, generating the prompt to the User.
To get rid of the prompt and resolve the issue to be able to relate emails via the Gmail Integration side panel, you should revoke the tokens from the User page and delete the Gmail Integration from the list of authentication providers in the org. To do this, follow these steps:
First from the Gmail Integration control panel, log out the User and close your Gmail tab.
In your Salesforce org, follow these steps:
Switch your Salesforce user interface to classic interface i.e. https://nimb.ws/M81XEd
Click on the Setup option.
On the Quick Find box, enter Users and click on Users from the results.
Locate the affected User (your own user) and access the User's detail page.
Under the 'Third-Party Account Links' related list, revoke ALL the 'Gmail Integration' and 'Gmail Integration with Inbox Auth. Providers' for this User.
Access Auth. Providers in Setup menu.
In Salesforce Classic: Type auth in the Quick Find box and click on Auth. Providers under Security Controls.
Setup | Administer | Security Controls | Auth. Providers
In Lightning Experience: Type auth in the Quick Find box and click on Auth. Providers under Identity
Gear icon | Setup | SETTINGS | Identity | Auth. Providers
Click on the Del link next to Gmail Integration.
After deleting Gmail Integration from this list, the User should go back to Gmail, launch the 'Salesforce' add-in if it's not already loaded and connect it to Salesforce, then follow the prompts to set up inbox and then connect to their Gmail account again. Gmail Integration will not prompt for permission to access the user's Google account anymore.
If issue is not resolved after following above steps, then try to switch OFF and then ON the Inbox in the Gmail Integration setting from:
In Salesforce Classic
Setup | Administer | Gmail Integration and Sync | Settings
In Lightning Experience
Gear icon | Setup | ADMINISTRATION | Email | Gmail Integration and Sync
Once it's done, go back to your Gmail and try to use the Salesforce add-in.
