field updation of present address

I want to update my field whenever a customer change his/her present address and it will send the notification to that customer using email alert.

 

But here i'm not getting what to mention in the formula field.

 

Can anyone please help me??

    i have not creatd sub fields... all d fields which u hv mentioned here is in 1 field. According to that please help
