Custom as in build from scratch, or custom as in use the Approval Process piece that comes pre-built with Salesforce?

I would never extend to a custom approval process until I was sure that the standard one didn't fit my needs. Typical use case for a standard approval process would be to request a discount from your boss to close a deal
