When converting leads, the tool presents you with a picklist where you can choose an existing account, preventing duplicates from being created. Unfortunately, we have not found a way to defulat this picklist to one of the recommended values so it has been a training issue for us.
On managing the single account, we are using Account Teams. This is standard functionaity which is displayed as a related list on the Account and allows us to list all of the users who are selling to or managing the account. It also works well if you have a strict default security model set in the Org Wide Defaults.
