Call Log Field with Date and Time Stamp

I have hired an inside sales person and as a result started looking at some call center apps on the appexchange.  Most of them are overkill but I have seen a couple of things that would be helpful.  One of them I saw in Powerdialer is a call log feature that allow you to put in what happened with the call.  But when you update the lead it puts the information in a text box with a time and date stamp.

 

Can anyone tell me how to set up a custom field like that?

    Ben, I appreciate the quick response.  I am new to Salesforce but I will take a look at it and see if I can figure it out.
