there isn’t a built-in way to digitally sign them directly while entering the information. If a signature is required, people usually handle that by using an approval process, a custom checkbox or date field to confirm sign-off, or by integrating an e-signature tool like DocuSign or Adobe Sign.  

For taking notes in Salesforce, you can use Salesforce Notes
