I couldn't find exactly that, but I did find this:  

which has some great content, and one thing I will definitely use the next time I'm building/leading a team - the working styles survey. You can download it from the resources - downloadable activities guide, on this module.  

Has anyone else in this Group seen anything helpful in Trailhead or elsewhere?
