Is possible to customize the out of the box NPSP Email Opt Out functionality with our own pop-up messaging?

I think you may need to disable the tdtm class: CON_DoNotContact_TDTM, instead you can create a custom popup Lightning Aura Component to show a message popup as you need, or maybe launch a flow.
