The first should fire when either the Accepted or Declined checkboxes are checked, and EITHER of the Assigned Date and Assigned By fields are empty. Right now, the rule fires when both are empty, but stops firing when one is filled in. Here is the current formula:
AND(

Accepted__c = TRUE,
Decline_Referral__c = TRUE),
ISBLANK( Accepted_Declined_Date__c ),
ISBLANK( Accepted_Declined_By__c )
)
The second rule criteria are:
Direct_Engagements__c > 0
Or
Events_Attended__c > 0
Then at least one of the below must be populated:
CCMI_Contact_Assignment__c
Parent_Partner__c
Work_Based_Learning_Assignment__c
(I'm not sure where to start with this second formula).
