Data Import wizard and Field Mapping

For the data import wizard. I went through settings-> import-> and  I got to the"Edit Field Mapping: Accounts and Contacts" page and I keep getting warning messages preventing me from moving forward stating "The matching field you chose (Name) is not mapped" "You need to map the required field Contact: Last Name."

 

"You need to map the required field Campaign Member: Campaign."

 

Can anyone walk me through how to fix these deterents?

    It looks like I am only importing contacts, phone numbers and email addresses and I believe that I just want to add and not update.
