Dynamic Record Choices Options Not Showing

I am working on a flow that populates a dropdown list with the names of contacts that are associated with the current account. In the flow designer, the list populates just fine in debug mode and the record is created. However, when the flow is activated and added to the page view, none of the contact names populate. Any help would be greatly appreciated.

    I figured it out. Thank you Narender for your help. Realized that the syntax was wrong in the URL. Should have read: 

     

    /flow/Update_Request_Case?varCurrentAccount={!Account.Id}
