Custom Field not an option when creating a form

We created a new custom field called "DC Referred By" for a referral program, and you can see this new field on all pages (lead, opportunity, contact). When I go to create a new form, this custom field is not an option to map to. What am I missing?

    Where are you creating the form?

     

    Is the form Vf page or something else?
