The first part is easy, as Preethi mentioned.
The second part is not easy to do. If you just need to do this from time to time, you can run a Lead History report (make sure you are tracking history on Owner field). You'll be able to see when Leads changed owner, and you'll want to look for ones where the old value = the queue you're interested in. Unfortunately you cannot filter by old or new value in a Salesforce report, so if you need exact numbers, you'll have to export to Excel and do your filtering there.
An option I have considered before, but never ended up implementing, is as follows. You could create a field on Leads called something like "Previous Owners". Every time a new owner is assigned, have a workflow rule update the field and add the name of the user or queue to the field, maybe separated by a comma. So if a lead started in your Marketing Queue, then went to the Phone Sales Queue, then went to Field Sales Queue, then got assigned to Jon Smith, the workflow rule would update the field each time, and the results of the field would show: Marketing, Phone Sales, Field Sales, Jon Smith. You could then report on whether that field contains the name of the queue you are interested in, which would tell you it is currently, or was at one time, assigned to that queue.
