Is it possible to change a manager name to display his role instead (ie: Area VP West, vs having his name "John Smith")
It would be nice if the VP could see the hiearchy by area vs person managing the area. Is this possible in some setting?

No thats currently not possible in Salesforce.
However there are couple of options that you can look at :
1) Create A Custom Report Type as Forecast Report and from the Custom Report Type layout, drag the Role of the Owner for Grouping.

2) You can directly View information of a forecast from an end user's prespective. You can select a specific user and see forecast from his view.
