The formula works if I select Deal Type as Quarterly, Semi-Annual, but if I select Deal type as Monthly, or Annual. the Year Text and the Opp name details field do not populate on the opp name.
Any ideas why that would be the case?

Can you duble check that you don't have duplicate WFR running?
Also can you post the complete Formula and a screenshot of the WFR and the Field Update Action?
