I am trying to do this without listing all of the other countries, but I see nothing that initiates a validation rule with a "Not equal"

I hope this doesn't seem like a silly question, but are you absolutely sure that the picklist option for the account billing country code is "United States" spelled out and spelled out correctly? If it's US or maybe there is a mispelling in the picklist value, that could be causing this. I just ask because I created a test of the last formula I suggested in one of my org's objects with two picklists and it worked perfectly.
