
We have identified the issue as stemming from the Salesforce for Outlook tool. We figured this out by disabling plugins until finally turning off Salesforce for Outlook and letting him use his machine for the whole day to see if it would still occur. It finally stopped happening.
What I don't understand is why this is happening, and only to 1 of 26 reps on our sales team. Every other rep is using the same tool/plugin and has a configuration setup pretty much identical to each other. (I create individual configs, instead of one master config, in case reps want to modify their settings).
Has anyone experienced a similar issue, and if so, how did it get resolved? We really leverage off of that tool, so simply having that rep not use it, is pretty much not an option.....

I tried several things with this user. In the end uninstalling all Salesforce products from Add/Remove programs took care of things. Here's what I tried in order of what was successful:
1. Removed Salesforce for Outlook, Salesforce for Office and some other Salesforce for Outlook application from Add/Remove programs.
2. Deleted c:\users\<username>\appdata\roaming\salesforce.com and \.salesforce.com folders (that second folder has a '.' dot in front of it) (this was a Win7 user); non Win7 users should delete c:\....\application data\salesforce.com
3. Reinstall SF4O and reconnected to our configuration and he was fine, no more blank appts and solid syncing
4. Additional things that may or may not have had anything to do with this resolution:
- deleted and recreated users local Outlook email profile
- deleted all contacts from SFDC for user
- deleted all events and tasks from SFDC for user
