We are just about ready to roll out Lightning to a small pilot group of test users. Looking for suggestions for questions that will help guide the pilot testers to provide enough detail on issues they may encounter and/or changes they'd like to see. Here are some examples:

• What is the issue?
• Where is the issue located?  (which object? component? or location on the page?)
• What happened?  Describe the exact steps you took that led to the issue?
• Was there an error message?  
• What should have happened? i.e. what did you want / expect to happen?
• Is the change limited to a specific object/location, or does it need to be implemented in multiple locations?

Any other suggestions, or thoughts?
