I can't believe I didn't really notice this before. The Gender field installed by NPSP (Gender__c) and used by default by the NPSP reciprocal relationships has settings for Male, Female, and Other. But those are sexes, not genders. If you want to actually record the gender identity of someone, I think you would offer Man, Woman, etc. I recognize that the NPSP field would be very difficult to change at this point, particularly since it's tied in to the Relationships settings. But it seems like we might want to offer something a little more in line with identity usage. Perhaps a new field that can have the right picklist values and then a flow that would keep the new fields' choices in line with the existing field's options for the relationship settings...?
