Create Custom Button for "Add Attachment"

I'm trying to create a custom button on the Case object that will allow the user to add an attachment.

I can't seem to get the Button URL just right:

/p/attach/NoteAttach?parent_id={!Case.Id}&parentname={!Case.CaseNumber}&retURL=%{!Case.Id}
