When i am trying to create the case its now allowing to add the required details.

@Sarita Adsare, review my first comment ; i am mentioning the specific layouts of your custom actions (quick or global). See the screenshots @Archana Panda has added to the screen. That are those (empty by default) custom layouts that are displayed when accessing your custom actions, and not the overall generic layout available on setup>object manager> select your object > Page Layouts.
