Forecast report showing wrong values for Forecast Amount, missing data

I've set up collaborative forecasting and everything is working fine in the forecasts tab. The report I've created, however, is showing wacky values in the "Forecast Amount" column that don't correspond in any way to the Opportunity Amount. I'm seeing wrong values and repeating values. Also, here are some Opportunities (including whole Forecasting Item Categories) with missing information.

 

Any idea what could be causing this?

    I am using a custom report type Forecast Quotas with Forecast Items with Opportunities. I assume folks are using a similar report type. It is otherwise an incredibly useful report showing the opportunities rolling up to your quota. Unfortunately the Sum of Forecast Quota is wrong.  

     

    If you look at the Quota for  individual records, it is correct 95% of the time, but then the Quota switches to $0 or a random number on %5 of the records. This means that the SUM/AVG/MIN/MAX all report a number that is not the quota.
