Creating a custom pardot preference ceter with two pages where users can enter their email address and view their preferences

I am trying to build a Pardot Preference Center where the user will enter their email address on page1 and they will be taken to page 02 which will display the preferences they have been opted in previously and they can opt. in and out from this screen. They should also be able to unsubscribe from all lists from this list. How do I do this?
