how to add custom field to salesforce 1 form

Could you please elaborate on the requirement? 

1. Where exactly in Salesforce1 do you want to see the field? Can you post a screenshot from your Salesforce1 to show that?
2. In your screenshot above, do you not see the custom field that you wish to add?
