How can i create a field that will pop up like this? what i want to basically do is if the stage is set to closed lost then i want a text field for a reason to be written. I would like this to be pulled up in a new window like the one above. Can anyone help with this?

If I understand you correctly, you want some pop-up window to appear. I did something like this on visual Force page, I think the simplest way to implement this, it using some javaScript(jQuery UI will be good for this)
