Go here http://certification.salesforce.com/ and click the "Register for an Exam" button. You will need to create a Webassessor account. Then go through the start of the registration process to select an exam type and on site testing. Then it should give you testing center locations. You don't have to complete the registration until you are ready to actually schedule.
