Hi Antoine,
You can use third-party tools like GetFeedback or FormAssembly to create a survey (which would be in the form of a poll) and have them update their Contact data through that form. Otherwise, you would need to either have a Salesforce Community where they could just update their contact details or create a visualforce page with those fields and send that out.
