is it possible to change page layouts with the pre-configured npsp?

or do I need to use the "clean slate" version. Specifically, there are additional fields I need to add and capture when a new contact/account/opportunity get added.

    Yes it is.   Check out Setup->Customize->Accounts->Fields to add new fields and Setup->Customize->Accounts->Page Layouts.
