Introduction

Care programs are provided by life sciences and healthcare organizations to support patients on their health journeys. Their goal is to increase adherence to care plans, encourage wellness practices, and provide financial help. Care programs can cover services such as patient outreach, access to medications, access to care, financial assistance, or remote monitoring. The Care Program data model helps you capture those activities so you can use Health Cloud to plan and track them better.

Explore the Care Program Data Model

As part of Charles’s care plan for lower back pain, Dr. John Bostworth, his primary care physician, prescribes him a specialized pain medication. Dr. Bostworth also got Charles’s consent to enroll him in the GoodHealth Pharma Pain Management Care Program (Care Program). This program is intended for patients who have been prescribed GoodHealth’s Pain-Away medication (Product) for pain management.

GoodHealth Pharma Pain Management Care Program

The program goals (Care Program Goal) include helping patients manage their pain by adhering to the prescribed medication dosages and incorporating nonpharmacological strategies. The Pain Management program includes education on pain management and pharmacy delivery program as a service. The program also includes alternative pain management strategies as part of an educational campaign (Campaign).

As part of his enrollment, Charles completed an assessment (Survey) to help the team proactively address potential risks, challenges, or opportunities that can impact Charles’s health outcomes.
