This gear icon has been missing for 3 days now - is this a lightning update? I know that there is a button at the bottom of reports now that says "detailed rows", but this only expands one box of the report at a time.
How do I get the gear icon back on my reports?

I think you are seeing the new Enhanced Run Page (https://releasenotes.docs.salesforce.com/en-us/winter19/release-notes/rn_rd_reports_enhancedrunpageui.htm). That "Detail Rows" toggle at the bottom should behave the same way as the old "Show Details" option under the gear icon. If it's not, can you take screenshots showing the behavior of that "Detail Rows" toggle?
