Enable Multiple Custom Care Plans
- Describe why multiple care plans are useful.
- Set up Health Cloud so that it can run concurrent care plans.

Leif Hansen knows that in Health Cloud, you can create and execute a care plan for each patient. He knows that Health Cloud creates this care plan for each new patient added to the system. He would like to extend this mechanism to create care plans for his patients’ memory issues. He asks Salesforce admin Harryette Randall if it's possible to create a new care plan type called Memory Plan. With a care plan type like this, Leif can focus his patient management on specific conditions related to memory loss.
Fortunately, Harryette already has looked into this very question, and she confirms that it is possible. She shows Leif how it works.
Care Plans in the Health Cloud Console
Harryette explains that when a patient has multiple care plans, they all appear in a condensed view in the Health Cloud console. From there, Leif has easy access to the underlying problems, goals, and tasks of each care plan.
The Patient Navigation menu (1) provides access to manage any of the patient’s open care plans.
In the All Care Plans view (2), Leif can see and manage the details of different care plans.
And, when he needs to, Leif can expand an individual care plan and see its details (3). He also can open a care plan and work on it in a tab.
As caregivers add new care plans for a patient, the plans appear in the order created, with the most recent care plan appearing first. Harryette explains that she can create custom care plan record types to enable different kinds of care plans.
Following this show and tell, Leif asks Harryette to add a Memory Plan care plan type.
Create a Custom Care Plan
The first thing Harryette does is enable multiple care plans.
- From Setup, enter Custom Settings in the Quick Find box, then select Custom Settings.
- In the list of custom settings, select Manage next to the Health Cloud Feature Toggles.
- Click Multiple Care Plans.
- Check that the checkbox next to Enabled is selected. If it isn’t:
- Click Edit.
- Select Enabled.
- Click Save.
Then she prepares to add the custom Memory Plan care plan type.
Harryette knows that it’s important for specific users, like Leif, to have permission to access custom care plan record types. So she makes sure that any cloned permission sets include Read and Edit permissions for the Care_Plan_Record_Type_Name and Care_Plan_Record_Type_Namespace fields on the Lead object. To set up the permissions, Harryette gets a list of her cloned permission sets and then updates them.
- From Setup, enter Permission Sets in the Quick Find box, and select Permission Sets.
- Click the name of a cloned permission set.
- Click Object Settings, then Leads.
- Click Edit.
- Locate the Care_Plan_Record_Type_Name and Care_Plan_Record_Type_Namespace fields and select Read Access and Edit Access for each field.
- Click Save.
- Repeat steps 2 through 6 for each cloned permission set, until you’ve updated all of them.
Now Harryette adds the Memory Plan record type.
- From Setup, enter Custom in the Quick Find box and select Custom Metadata Types.
- Next to Care Plan Record Type, click Manage Records.
- Click New.
- Provide the following values and settings:
- Label: Memory Plan
- Record Type Namespace: the org’s namespace, if it has one
- Case Record Type Name: Memory Plan
- Active: Select
- Default: Deselect
- Click Save.
The next time Leif visits a patient's record, he can open the All Care Plans view and, from New Care Plan, add a memory plan.
Leif is happy with his new Memory Plan record type, but Harryette lets him know about a couple of the features that he could ask her to implement in the future. She can:
- Customize the problems and goals that come with each care plan
- Create care plan templates
Health Cloud provides Salesforce admins with a range of ways to modify how patient information is displayed. By tailoring these displays, medical professionals and caregivers can easily maintain and see the information they need to provide optimal healthcare.
