The Health Cloud household data model represents the relationships between patients or members, and the people and businesses participating in their care. This flexible data model lets you manage connections among any combination of people and organizations. You can create relationships between contacts, accounts, and accounts and contacts. Creating these relationships makes it easier and more efficient for the care team to provide the appropriate care that the patient requires.

Here’s how the household data model represents the caregivers and organizations that support Charles Green’s healthcare journey. This includes Account Contact relationships, Contact-Contact relationships, and Account-Account relationships, all associated with Charles’s care.
