Introduction

The social determinants of health data model represents the critical social and environmental factors that become barriers to a patient’s or member’s wellness. This data model allows you to gain insight into the physical environment and socioeconomic status of a patient, and obtain a holistic perspective on factors that influence wellness, adherence, and outcomes. This helps you identify intervention tasks to address barriers and help reduce health setbacks for patients and members.

Explore the Social Determinants of Health Data Model

As part of Charles’s enrollment in the pain management program, he received a pain management assessment survey (Survey). This survey helps the program team in proactively addressing potential risks, challenges, or opportunities that can affect Charles’s health outcomes.

Since patients can have a hard time getting to pharmacies to pick up their medication or going to doctors appointments to receive important care, access to transportation is considered a determinant of health. Some of the questions in the survey pertain to Charles’s access to transportation (Care Determinant). Some of Charles’s survey responses (Survey Response) indicate that he doesn’t have a car (Care Barrier) and he can’t pick up his prescriptions on time.

The Good Health Pharma care program team logs a case (Case) to find a ride service (Task) covered by Bloomington Health Plans, Charles’s health insurance provider. By securing transportation (Care Intervention) for Charles, they’ve helped him successfully overcome his transportation barrier. He’s now on track in his journey to better health!
